If you have a personal story or scanned photographs you would like to share for use in newborn screening education, awareness efforts or for the website, please read all information on this page, and follow the directions below:

Before we can use your story & photos please get a Release Form on its way to us. If you have any problems with this form contact us so we can send it to you in an alternate file format, or by USPS.

Complete the release form and return it using the information at the bottom of the form - (via fax, email, or USPS).

Then email the story and/or photographs.

Need Your Photos Scanned?
If your photographs need to be scanned by us, please mail your photos along with a completed release form. If you would like your photos returned, please include a self addressed stamped envelope.

Photos by Professional Photographers
Photographs taken by professional photographers can not be used unless you include a signed release from the photographer.
